Critical Incident Reporting System (CIRS)

In many hospitals, errors, injuries, and damage to patients are not openly discussed and learned from. Causes of errors can be detected and removed through the development of an appropriate error prevention culture, through the creation of appropriate general conditions, and by installing a Critical Incident Reporting System (CIRS).

Training Goals • Understand types of errors, their effects, and their causes • Understand requirements for an error prevention culture • Understand the benefits of developing a Critical Incident Reporting System • Understand the basics of developing, implementing, and maintaining a Critical Incident Reporting System
